NURSING AT HOME
There are many reasons why you may realize the supports of a nurse in the comforts of your home. Some of those reasons may be a disability, an illness, end of life care, wound care, poor health and recovering from surgery or hospitalization. Maybe you would benefit from help with changing wound dressings and ensuring that your medication is taken.

Our nurses both Registered Nurses (RNs) and Registered PracticalNurses (RPNs) are regulated, screened and bonded. Our nurses are available 24/7 toensure you receive your needed and desired care in the comforts of your home. Weoffer end of life care, wound care, senior wellness care and post hospitalisation homecare.
If you think that you or a loved one would benefit from our Nursing At Home Service then please communicate with us. We will visit with you in your home to get acquainted with you and the layout of your home. After consulting with you we will custom design a Care Management Plan for you.
END OF LIFE CARE
At the end-of-life phase, you will have various decisions to make.
Where will you like to spend this phase? Our team of compassionate caregivers and nurses will give you the agreed-upon care you need and desire.
This care can be genuinely carried out in the familiar surroundings of your home or a care facility. This care can include services from Nursing, Personal, and Home Care services. Our care team will treat you with the dignity you deserve through this phase. The caregivers will carry out your medical treatment decisions while also respecting your spiritual and cultural preferences during this time. We will give you comfort and be sensitive to you, your family,
and your friends during your end-of-life care.
If you think that you or a loved one needs our End Of Life Care service, please communicate with us. Our Care Coordinator’s will visit the home of yourself or a loved one. We would then get acquainted with you and any family and the layout of your home. After consulting with you, we will custom design a
Care Management Plan for you.
WOUND CARE
Proper wound care prevents infection, reduces pain, promotes healing completely and quickly as possible. It also advances the healing process and with less scarring. Maybe you have chronic scarring, post-surgical scars, and diabetic ulcers/wounds. Our qualifiednurses will implement the appropriate care. If you need our Wound Care service, please communicate with us. Our Care Coordinator’s will be happy to visit with you in your home or care facility. We look forward to getting acquainted with you. After consulting with you we will design a customized Care Management Plan for you.
SENIOR WELLNESS CARE
Are you being more forgetful of things that were simple. Did you leave the stove on, almost causing a fire for the second time in a week? Maybe you forgot again to lock the door of your home? Are you experiencing pain during certain movements or even when not moving? Are you reacting irrationally more regularly? These are examples of wellness depreciation that you may be experiencing now to some degree or may in the future.
If a person experiencing this wellness depreciation had a nurse conducting Senior Wellness Checks with them on a regular basis they would benefit in a number of ways. The nurse would have been able to discuss initial symptoms and their realized and potential harmful results to themselves and others. Any family and concerned friends would have been made aware so they could respond in a helpful way. Also, a visit with their doctor would have been arranged to discuss their symptoms and how they could have been treated and safeguards put in place.
This is why FRESHSTEPMED provides Senior Wellness Checks by a qualified nurse on a regular basis to you in your home if you desire. The nurse will check and assess your wellness levels that can change through health conditions and simply aging. The nurse will check many of your levels such as, hydration, eating healthy and regularly, medication management, vital signs, your physical mobility, and mental and emotional health. POST HOSPITALISATION HOME CARE
You’re on the way home from the hospital after a serious operation. Unfortunately, your mind is thinking of all the adjustments you will have to make in your home to help you peacefully recover. Maybe you will need specific medical equipment to help with your healing, mobility stability and comfort. Equipment such as an adjustable bed, commode, walker, and wall support grab handles. Maybe you will need bandages changed, intravenous administered, and help with personal care like getting dressed and showering. How will you get meals prepared and your home kept clean?
Fresh Step Med wants to give you guidance, care, and support to help you with your adjustments from the hospital to coming back home. We have qualified personal support workers (PSWs) nurses (RPNs & RNs), companions, and cleaners. It would be very helpful for you to preplan for this Post Hospitalisation Home Care if you think that some adjustments may need to happen in your home after an upcoming hospitalization time.
